This form is for use by members who wish to withdraw all funds from their credit union account.

NB: Doing so will result in the termination of your Credit Union Membership

Important:

If you submit this form your account will be closed

You should understand that this means:

1. You will no longer be a member of this Credit Union
2. You will not be entitled to any dividends announced after you end your membership.
3. You will no longer have access to Credit Union savings or loans.
4. If you are retiring or leaving BAE Systems employment, you understand that you do not have to leave the Credit Union.
5. You cannot end your membership if you have any outstanding loan balance.
4. If you have left BAE Systems, you will not be able to re-join in the future.
